  3. Operating Agreement
  4. Roster of Members
  5. Lien Certificate or Letter from Acct.
  6. Form SS-4 with EIN Number

All members must sign (or provide proof for all necessary consents and authorizations), unless LLC is manager-managed, then manager(s) must sign.
